To answer these issues.
1. The sounds not working in the preferences pane is part of the general
issue with notification sounds, same code path and once they stop working it
also stops in the prefs pane. Also the startup sound does not cause this
problem to occur, see my other mail for all the ones that do.
2. Correct about the link at the bottom of the page, I will pass this along.
3. This is populated by sound sets found in: ~/Documents/Microsoft User
Data/Entourage Sound Sets

these sounds, no sounds will ever be played again. So...the sounds are there
and you can hear them...until the first sound is triggered.)
