How do you sort contacts in Entourage 2004 (operating system is OS X)
by category then by name? When I click / sort on the name column it
sorts fine alphabetically but when I click / sort on category column
the name column no longer sorts alphabetically and the names are now
listed randomly.

Click on the Name column then in the popup menu (top right) select Category.
