Error: Could not synchronize record

I have suddenly begun getting the following entry in the error log:
Could not synchronize record: Review ' introducing RealMed' document
to Exchange server: RealMed

"RealMed" is the name of the company I work for. I have no idea what
this error means but it's constantly filling the error log with
multiple instances. I have searched through my folders and cannot find
any document with the words "introducing RealMed" in the title. Weird.

Any help will be greatly appreciated.
