There seems to be something wrong with your database, I suggest you start
with a fresh copy (expand the files IAS.MD_ and DNARY.MD_ from the windows
CD to IAS.MDB and DNARY.MDB) and copy them over to %windir%\system32\ias
folder

(You will lose all configuration in the older databases by doing this)
