Broken image on Internet, no broken image on intranet

I'm running Windows Server 2003 Standard with IIS 6.0 and php installed.
We're using the server to host a website, but we're having a problem.
When browsing to a php page on our local intranet, our pages look fine,
with no broken images.
However, when testing this remotely over the Internet, most (but not
all) of the images are broken. If I right-click on a broken image icon
and select "View Image", I can view the image, but the image is still
broken when I refresh the page.
When developing and testing locally, the php configuration uses the
local ip (192.168.x.x) address, but when testing remotely, it uses
www.mydomain.com. The only port of the server accessible to the web is
port 80.
I know this isn't rocket science, but I'd appreciate any tips!
