Index.shtml now asking for authentication! Help!

Here's my setup: Win2K Advanced, IIS5. My index page is
index.shtml. It contains a Server Side Include. The web
is open to the public with anonymous/integrated windows
security. Everything worked fine until a reboot last
week. Now, everyone is getting prompted for
authentication to the index page. Then they get a 401.3
message talking about ACL on Resource. Thinking it might
be permissions, I gave the IUSER_WEB account full READ
rights on the whole web root. But the problem remains.
Also, I tried renaming the index.shtml file to index.html
and setting index.html as the default document just to
get the web back up and this works. People can now view
the index page with no problem. However, the include
component isn't displaying on the page. I've checked
and made sure that ssinc.dll is registered in IIS Manager
to handle .shtml files and checked NTFS permissions as
much as I can tell. Does anybody have any ideas? PS-
I'm runnin Service Pack 3 and maybe upgrading to SP4 will
replace some corrupted files??
