FYI Dave S indicated uninstalling Norton AV fixed the problem.
I had a similar problem which was fixed by exluding certain directories from on access anti virus scanning.

As I said, if I reboot the machine it works once, but it takes a reboot. Restarting IIS or SMTP service doesn't work by itself.
