Microsoft on a tech support call has confirmed that on access anti virus scanning products can create problems between IIS SMTP and Exchange.

For whatever reason they choose not to post this in any KB articles.

The problems will be intermittent or can start up out of the blue. AV tools get updated, not only the dat files but the engine itself get updated, hence it is possible for thing to work one day and not the next.
