XXX BHO caused error and closed IE - is that accurate

I have a simple BHO which traps keyboard strokes using IE level
Keyboard hook ( As i wasnt able to do it properly with any other
ways) and automatically fills form fields of few urls.

One thing i noticed is that very few percentage of users ( 2%) once in
a day get error messages like XXXX BHO caused error and ur internet
explorer need to close. And Per them , this started showing up after
installing my BHO ;)

XXX mostly is mtwbho.dll , which is actually Keyboard Macro Program
from MacroTool Works ,but today one user reported the same behavior
thrown by Flash Addin as well. I was wondering if the messaging by IE
accurate or not.

Is there a chance that actual problem is in my bho and the messaging
is not accurate
