Installing a toolbar when a browser is open -- SetSite fails, why?

Hi,
At installation, the installer checks if a browser is open.. if so closes
it,
then proceeds to install the toolbar. After successful installation,
I launch IE but though it shows the new toolbar as "checked" in
View|Toolbars, it is not shown nor visible on the browser. It's not
obscured even. You must un-check, then re-check before the toolbar
shows up.
I debugged and found out that the first call to SetSite received an
IUnknown interface that doesn't have a parent window and so
the toolbar is not created.
Question is: Why would SetSite fail in this case? What are the
instances that SetSite may fail? What should I do in order to
avoid this.
