It only works if:
1 - the "external" user has a direct (non-NAT) Internet connection
2 - the "external" user initiates the AV connection
3 - the "internal" user has the firewall client installed
4 - the "internal" user has configured the IM client to use "direct" (non-proxy/socks) connection
