Http Status Code - Red ? and 4 ????

Hi Folks;
I'm using ISA 2004 and looking through the log files monitoring http
requests.
In the HTTP Status Code column I'm seeing an X in a red circle along with 4
question marks when a request has been denied.
Does anyone know what that means exactly? Or what caused the request to be
denied?
