From your post, you said "The connection is using LDAPS, not connecting to

this is a customized form set." It seems the successful logon audit has
been logged in ISA. Therefore, we think the changing password behavior has
been completed. At this moment, could you logon with the new changed
password to log on the form?

Because the form is a customized form set, it is hard for us determine if
the issue is related to the customized form. Following articles are for
