I wonder what is the DNS settings of your ISA server interface(s)?
It's crucial part of any of the split DNS infrastructures to have properly
configured DNS settings both on client machines and on the ISA server
itself.

Follow-up to this: After reading more articles, I unchecked the
"Require all users to authenticate" on the Internal Network properties
and removed "All Users" from the "Unrestricted Internet Access" fiewall
rule, leaving only "All Authenticated Users".
Now the IE client receives the following error when trying to access
www.companyname.com: "Cannot find server or DNS Error
Internet Explorer".

Receiving the following error: "Error Code: 403 Forbidden. The ISA

when "Require all users to authenticate" is enabled and proxied IE
users attempt to access a web site with the same name
(www.companyname.com) as our internal AD domain name (companyname.com).
Access is allowed if users are not required to authenticate, or if they
do not proxy through the ISA 2004 server.
A DNS A record maps www. to external IP of web site.
Any thoughts on how we can enable username tracking and still allow
access to this web site?
Thanks.
