Usually, if you want to use ISA to control outbound Internet access as well
(i.e. keep a tight leash on the internal users), it's recommended to join it
to the domain so that you can take advantage of the full authentication
capabilities. If you have a relaxed environment where everybody have access
to everything, that kind of thing, or if you're only using ISA for server
publishing (unlikely but possible), then your best solution is to keep it
outside the domain.

Not really. RDP is a great option for administering your ISA server,
especially when coupled with a VPN connection to your ISA.

Yes there is, ISA ships with a default "system policy" which is a collection
of certain firewall rules. In addition to the rules you're going to create
on your own, you will most likely have to customize the system policy for
your purposes.
