Thank you for posting.
Based on your description, my understanding of your network structure is as
following:
LCS 2005 - ISA 2004 (front) - DMZ (Access Proxy) - ISA 2004 (front) -
Network A - Internet
You had published the SIP on the ISA 2004 (front) server and a cient
computer located in the Network A can connect to the Access Proxy without
any problems.
This issue is when a client computer connect to from Internet is a VoIP
NATed client, the Voice Communication cannot work.
Since the client computer located in the Network A can use LCS correctly,
it means the configuration in ISA is correctly.
There is no problem regarding the ISA part at all.
If the VoIP NATed client cannot connect correctly, you will need to verify
the your networking between the ISA and the client computers.
To this issue, I recommend you to check it with your ISP and the client
users to check how the connection was made.
Something between the ISA server and client computers block the connection
to the 5601 port that you had published.
