I have a client with a similar problem. Outgoing TCP connections are lost on the WAN NIC, but incoming VPN still works. Re-starting the SBS server corrects the problem. Other symptoms are, pinging external IP addresses cause a Destination Host Unreachable message and incoming TCP connections fail on ports that are allowed. I suspect that the Server needs more disk space on the system drive as I have caught a few errors with Exchange and Web Proxy that point to lack of memory. I suspect that when a ISA rule is broken (i.e. port scan detected) ISA somehow pulls down the curtain and all connections are lost except for incoming VPN using PPTP.
