Need help with publishing....

I have an application that is web based that I would like to publish. To
connect to the application internally you connect by
http://app.domain.com:58080/admin. How would I go about publishing this? I
am new to ISA 2004 and didn't know if this would be considered server
publishing or web publishing. Could someone give me some tips on how to
publish a site such as this?
