do you use integrated NLB from ISA Server? Did you change any settings in the NLB MAnager from Windows?
I let the ISA console handle everything - none of the NLB settings have been changed manually via the NLB manager.
Actually I think I've figured it out with the help of Mr Shinder. See here:

Since our clients are Firewall Clients first and foremost (although they are also SecureNAT clients), the firewall sessions will be tied to a particular server.
We could get around this by converting everyone to SecureNAT (but losing the ability to authenticate), or Web Proxy using the routing script (but losing the ability to authenticate other traffic).
Does that seem correct?
