Form asking for Username and Password for approx 5 of users?

Hi,

I have a webform (created with FrontPage 2003) that seems to be
working fine.

The form writes the data to a file on the server (_Private folder) AND
emails it. 95% of users have no problem and they fill out the form and
all happens as it should.

A couple of users have reported that when they press Submit, a dialog
comes up asking for a username and password. There is no username and
password required. Nothing happens if they hit OK or Cancel, except
the form blanks and a white page appears. The data is not sent or
written to the file.

I cannot find anything wrong with the form (I suspect there is nothing
wrong with it as its working for most users) and it does not seem to
be browser specific. One user that reported the problem tried it in
both Firefox and IE7 and got the same result in both.

My tests using both Firefox and IE6 & IE7 have all worked perfectly.

Anyone have any ideas or thoughts on this?

It seems to be something local to the user but...

TIA

P.
.
