In order to use Front Page Forms (see Front Page help),
in the manner described by Bob, your site must be hosted
on a server that has the Front Page Extensions enabled on
your account. Furthermore, the account must be enabled
for email transport by your host.

In addition, you must publish your site using the "http"
method not FTP. Using FTP will "break" the Extensions and
render your form inoperative.
