I have a rough guide to setting up formmail.pl v2.9 obtainable at
scriptarchive at www.rxs-enterprises.com/tests/sendmail/

However, there is a better, more secure, script at
http://nms-cgi.sourceforge.net/scripts.shtml
The download package includes easy to understand configuration
instructions, with examples (though you will have to tell Windows which
program [Notepad] to use to open the readme files.).
This script should be uploaded and installed on your server as for
formmail.pl above.
