Make sure you publish your site using http://www.silverstreamcottages.co.uk
as the web address to publish to,
so that all of the necessary files for the forms are published.
If you have published using ftp:// you may have to have the host repair or
reset the FrontPage extensions, even though they do show as being installed
for your web.
