This is a simple form that should function without any problems using the FP extensions to process it.
The example actually uses PHP, but that can easily be changed to any server side language you wish - including the extensions (which would be easiest if you don't have the required programming skills).
Create a form in FrontPage, add your labels, text boxes, text areas and radio buttons etc.
Use Form Properties to send the form to a file (in the _private folder) and/or to email if your host supports email through the extensions (some hosts don't).
http://www.theenginerevolution.com/formtext.html will fail since you don't have FrontPage extensions installed on the website.
http://www.theenginerevolution.com/car-seat-covers-wallet-2.html does not exist.
http://www.theenginerevolution.com/formcgi.html - There is a problem with the Perl script at cgi-bin/custom.cgi.
Impossible to speculate on the problem without seeing the code.
http://www.theenginerevolution.com/form-copy.html Again - requires extensions.

It's exactly what I want, but the script (it's free) will take nearly all

I have spent 3 12-hour days trying to make a fp form-script work, I'm
ashamed to say and have finally managed to find this great forum again.
