HTTP 405 - Resource not allowed When trying to post form to SQL database

Hi, I have a form that I created in FP. It has two text boxes and file upload control. I have set up a database in SQL complete with a table and mapped all the fields. When I submit my form to the server from the FP preview, nothing goes into the SQL database and I get the following error message:
HTTP 405 - Resource not allowed
Internet Information Services

If I click the link in the error message to go to my home page, I get this:
HTTP 500.100 - Internal Server Error - ASP error

The object has an invalid ProgID of 'MSWC.MyInfo'.

When I test the form from my machine using IE, I get my "thank you for submitting...." page, but no data goes into the SQL server. Any suggestions?
