    I've seen this behavior before. In my environment it
    usually happens to users that work remotely quite a bit.
    One of the users has a habit of letting his password
    expire, so when it gets changed in AD his local laptop,
    using a dial-up connection 95% of the time, has his old
    password cached.

    We have a good dozen or so DCs, and one time I went to
    each of them and changed it on them all to get him by.
    After that I told him he needed to perform a password
    change while on the network directly.

    Getting him to log in directly on the network and then
    accessing the change password function seems to work, thus
    synchronizing the password properly on his laptop and the
    DCs.

    I think another time it may occur is if the user changes
    it while on a dial-up connection, but I haven't tried
    testing that possibility.

    I believe there is also a setting within Outlook which
    allows you to "request password" as well. It's not likely
    a user modified that setting themselves, but you never
    konw.

    I'd get as many details from your users about their
    password problem:

    Did they change their password recently?

    If so, while working remotely in Windos, remotely via OWA,
    or directly on the network?

    Are any intranet applications prompring them for
    credentials?

    Finding patterns of methods your users use which are
    common to all may help you track down what they are likely
    doing incorrectly.
