1. Typically people that send SMTP authenticated email are remote users such
as sales people on the road. Internal users typically submit their items
through the Exchange Store which does not follow the same path. Therefore
if you see large amounts of authentication for an internal user that is
sending email outbound I would investigate with this user to see if they are
intentially sending these emails out.

2. If you uncheck this item you will likely not be able to receive external
email.
