You don't have to have a public Internet address on the server. Your
clients can connect to an smtp server on the Internet, presumably
using NAT or a proxy or whatever. You can configure the smtp virtual
server, which is already in existence and ready on your server to send
outbound mail to your ISPs relay (ring and ask them if you don't have
its address)
Outlook will then be able to take all mail and throw it at Exchange.
Exchange will then be able to say "I know how to get out, and this
message is for out, great, off it goes"
No need for multiple profiles.
