I too have exactly this error (Exchange 2003 also)- at
first I thought it was because the domain we were having
problems with has it's primary mx record pointing to a
server which has port 25 blocked (the ISP does this to
stop open relays - mail servers then fallback to the next
mx preference which is the ISP's relay server, which then
forwards on the message as port 25 isn't blocked within
the ISP's net)..

This wasn't the case however - I pointed Exchange server
to a Messagelabs smarthost and we still have the same
issue..

There was one point this-afternoon when it worked for
about 10 minutes - right after I'd setup the smarthost and
restarted the SMTP virtual server - it soon stopped
working and no amount of restarting the virtual server or
deleting/adding the smarthost will get it working..

Does anyone out there have any further ideas/clues?

We aren't blacklisted in any databases..
