To make a long story short, we have a 2003 FE and multiple back ends
running 2000 and one on 2003. The 2003 BE works fine, the 2000 BE
servers (if they are using the front end for webmail) get a 404 error
when you click the change password button.
I read the KB article about checking for some inherited .dll files on
the virtual directory in IIS, but they weren't in there. The change
password button was working before I started using the 2003 FE for
webmail. If I bypass it, and go directly to the BE server OWA, the
password change works.

It seems to me like you've missed to map .htr files between Exchange 2K3 Front End and the back end servers.
