The cause of the problem is due to the Integrated authentication being set
on the Exchange(& maybe Public) virtual directories. Integrated do not
work well over multiple hops or through firewalls. The OWA authentication
settings should be as follows:

--Exchange & Public(set through Exchange System Manager console): Basic(w/
Default Domain = internal_AD_netbios_domain_name)
--Exchweb(set in IIS console): Anonymous

Users should, with the above settings, be able to log into OWA, internally
or externally, by just putting in their user name and password. When
accessing OWA over the Internet, we recommend using SSL.
