What name are they connecting with from the outside and the inside?
Maybe adding that name they are connecting from the outside and inside as
hostheaders may do the trick.
Confirm that the firewall is allowing it to pass thru by connecting to the
name without the /exchange part. You should get the default
underconstruction page (if configured with default settings).
