Hi,
1.) If you have audit account management turned on you will be able to find
who and when by looking in the security log on your domain controller.
2.) Create a new user (don't mailenable this new user) and from exchange
system manager reconnect this new user to the old mailbox. Make the new user
member of the same security and distribution groups.
