Internal, https://mysite.com/exchange works
External, https://mysite.com/exchange doesn't work, because the certif
points to email.mysite.com

How do I configure the default web site to answer for email.mysite.com?

Put an A record in the global DNS for "email"
Point that A record to the right IP address.
Job done.
