Your mail server will not receive email from the internet
unless you forward the proper ports through your firewall
and set up an MX record for the domain.
If you have it physicially plugged into the net you might
get randem spam from people scanning for open smtp ports,
but these will not be directed to any valid users.
If you find clients getting mail (actually in the outlook
client) they are likely bypassing your mail server and
directly hitting a pop3/http server over the internet. Try
blocking port 25 and 110 for outbound access. Only enable
IP ACL's for your mail server if you need that access...
Blocking HTTP mail required url filtering via some means.
