CA agreement practices

I'm looking for a best practice of someone that has migrated 5.5 to 200x
with respect to how CA's were used. My environment has all 5.5 servers
running on 2000 servers already part of a native mode AD. There are 16
sites and 13 child domains. We are also go to 2003 DC's as we roll. The
migration will take a couple of months. In testing, I noticed the CA wizard
wants to create CA's from the first 2003 server to all sites. What I'm not
clear on is whether it would be better to build the CA's site by site-domain
by domain as I migrate. Another conflict is the first connect set as a
primary seems to be random. If I need to take down a 5.5 server that get
migrated early in the project and it's a primary CA then I risk duplicating
objects from the warnings.
