After some more research I found the following post which
provided the solution to my problem:
----------------------------------------------------------
Dear Joe,

Thank you for the update.

If changing the password of IUSER and IWAM didn't fix the
problem, let us
check the following setting on the Exchweb\Bin virtual
folder. Please refer
to the following steps:

1. Start Internet Services Manager.

2. Locate the Exchweb\Bin virtual folder.

3. Right click the Bin virtual folder, and then click
Properties.

4. On the Virtual Directory tab, check to see whether the
application name
is "bin". If it is greyed out "Default Application", then
please click
Create and click OK.

5. Restart the web site.

If this modification doesn't help or the application name
is "bin" already,
let us change the application Protection on the
Exchweb\Bin directory.
Please follow the steps below:

1. Ensure the application name is not greyed out.

2. Under Application Protection Select "Low (Pooled)".

3. Click OK and restart the web site.

Can we log off properly now?

Please feel free to let me know if you have any questions.

Regards,

Jay Yan
