I'm so tempted to say it's the infamous Outlook Anywhere with Server 2008 IPv6 issue (see here: http://www.buit.org/2008/01/04/outlook-anywhere-is-broken-on-ipv6-in-windows-server-2008/), but that wouldn't explain why it works on the LAN.

You can hit https://<server_FQDN>/rpc from the WAN?

SPI has been turned off all along. What puzzles me is that OWA (via HTTPS)
works fine.
