For cause 2, you also have to verify that the correct permissions exist.

Our Exchange 2003 server stops sending mail via smtp, and the client
displays
"452 4.3.1 Out of memory"
Workaround: restart SMTP service on the server, but the problem reappears
after few minutes.
Windows Server 2003

Ref. MS KB329167:
Cause 1: Antigen antivirus is not installed
Cause 2: All Exchange/SMTP services run as Local System
Cause 3: BadMail folder is empty

Greatful for any help!
