We are also facing a similar problem with the exact same delay.
The initial symptom of the problems is a delay in the arrival of any
external inbound email.
After checking the Front-end Server, we noticed that the external messages
actually do reach this point, then a queue is created to route the message
to the Back-end Server, an SMTP session is established from the Front-end
Server to the Back-end Server.

However, at this point the SMTP Session in question will stay open for a
long time without actually transferring the queued messages.

Some of the delayed messages have arrived within 20 minutes of being stuck
in this queue, and some of them disappear without reaching the intended
recipient.

However, we managed to narrow down the problem to somewhere between the MTA
Service and the Information Store Service, which is causing the message not
to be delivered to the users' mailboxes in spite of actually reaching the
required server.

I've got an Exchange 2003 Enterprise server(Sp2) on a Windows 2003
Standard Sever(Sp1). What seems to be happening is that for 5-6 days
the mail server works fine. On the 6-7th day, the mail server starts
to slow down and mail becomes queued in the Pending Submission queue.
Any then takes approx 20mins to be delivered to Local users on the
same server.
