E2K3 SP2 + IMF -> SMTP Outbound reports errors if no smart host is set

I have a strange problem that I've not been able to put my finger on a
solution for and I've been ignoring it but now it's getting in my way...

I have an Exchange 2003 server running SP2. On that server is a single SMTP
virtual server. If I don't setup a smart host on the server I get messages
stuck in the outbound queues with random errors like "The remote server did
not respond to a connection attempt" and "The connection was dropped by the
remote host". If I put a smart host in place mail is transferred without
issue.

The smart host I'm using is a Postfix server that I want to get rid of that
I had been using for some spam filtering. However, I can't seem to get mail
to flow out of exchange directly for some reason.

Some of the things I've done are ...

1) I've verified that the DNS registration information for my domain is
correct.
2) I've manually walked the DNS queries that Exchange should be executing
(NSLOOKUP, SET TYPE=MX, yahoo.com...) Those resolve fine locally. DNS is
installed locally and Exchange is pointed to get DNS locally so I know it's
not connectivity to DNS and it's getting the right values.
3) I've manually connected -- and successfully delivered -- mail to the
remote mail domain. (telnet port 25 to the mail server.)
4) I've made sure that I'm not on any blacklist (that I can find.)
5) I've disabled the SMTP server and created a new one -- it exhibits the
same problem.
6) I've verified that my server has a reverse DNS record
7) I verified the FQDN being used (Advanced Delivery dialog) matches the
server's MX record.

Inbound mail always works fine -- and outbound works fine with a smart host.

One of the interesting things is that I just recently installed IMF and the
IMF node underneath ...\SERVER\Protocols\SMTP didn't show up although the
Global Settings\Message Delivery tab did show up. I redid the install a few
times thinking that I must have missed something.

Has anyone seen this kind of behavior before? Any ideas where to look since
I've verified DNS, direct SMTP delivery, etc...

Is there a way to force a reinstall of the SMTP components of Exchange? I
didn't see a way to allow SMTP to be reinstalled without completely removing
Exchange.
