Paul,
You will need to confirm your permissions for NTFS and Authentication Levels
on the Virtual Directories.
Take a look at the following article for the appropriate permissions.

One other thing that is critical to be able to manipulate items within your
Inbox is 'Read' and 'Write' access
to Public Folders.
The Virtual Directories are very important with regard to the ability to
access and 'draw in' the Inbox graphics.
Exchweb should be 'Anonymous' only and all structure under it need the
same. No Basic, No Integrated.
