I also have the same issue. I have a Windows 2000 AD setup with
Exchange 2003 enterprise running on Server 2003. I have discovered
this is the default for OWA access without forms authentication. So,
my Windows 2000 Pro users get prompted for a username, password and
domain name. Because I set my domain as the default, these users
don't have to enter the domain name. My Windows XP Pro users get
prompted for a username and password. But, because Windows XP handles
authentication differently, they have to use the domainname\username
format in the username field.

Once I get my SSL certificate and use forms authentication, I hope for
all this to go away.
