Here are some things to try

If you are using a version on Outlook prior to 2002, be sure Exchange Server
Service is the only service in the profile when you go to Tools -Services.
If Internet Email is in the profile, then remove this or create a new
profile with only the Exchange Server Service.

Try disabling client side anti-virus and check that this is removed as an
Outlook Add-in. If this fails try disabling Server Side anti-virus
temporarily.
