Mail Flow Issues

My scenario. (apologize in advance for long post, trying to get enough
details)

1 AD Domain / 2 sites (site A & B)
DMZ has edge server & ISA 2004

Each site has seperate Hub transport
Each site has seperate Mailbox server
Each site has seperate CAS server

Internet bound mail goes from Site A Hub through ISA to the edge
server and is delivered fine.

mail from the internet flows in through the Edge, to site A hub and in
delivered to the Site A mailbox server via MapiDelivery, works fine.

mail from the internet flow in through the Edge, to site A hub and in
delivered to the Site B HUB server via SMTP Relay to
RemoteActiveDeliverySite, works fine.

Intersite mail from Site A to Site B flows via SMTP Relay to
RemoteActiveDeliverySite, works fine.

here is where it gets sticky

Intersite & Outbound mail from site B SOMETIMES gets hung up.

I don't know why but SOME mail from site A to B flows via SMTP Relay
to RemoteActiveDeliverySite and gets hung up. SOME mail flows via a
SmarthostDelivery and works fine.

the error on the queue is
451 4.4.0 Primary target IP address responded with: "451 5.7.3 Cannot
achieve Exchange Server authentication." Attempted failover to
alternate host, but that did not succeed. Either there are no
alternate hosts, or delivery failed to all alternate hosts.

why might SOME mail flow one way, sometimes other?
I have permissions set exactly the same on each hub transport send and
recieve connectors.
I'm way confused, Please HELP!

there is a single IP intersite transport with both sites in it.
