Is there any documentation out there that explains the
process of moving the data from one machine to another?
I would like to read up on this, I could really use some kind
of step by step outline.
If you are willing to have the new server use the same network name as the old one (i.e. you're removing the old server for good), you can simply follow these steps:
- Stop all Exchange services on the old server
- Copy the Exchange database files to some network share or backup media
- Shut down the old server
- Install the new one and give it the same network name and IP address as the old one
- Join it to the domain
- Install Exchange using the /disasterrecovery switch
- Put the database files in the same disk place they were in on the old server (or restore them from backup)
- Start Exchange services

What are some of the pitfalls I need to be aware of?
You need to have the same level of service packs and hotfixes on the new server as on the old one, both for the operating system and Exchange. The best way to achieve this is to do a full Microsoft Update on the old server before shutting it down and then do another full update on the new server before restoring the databases.
Will there be any issuse with renaming the destination machine
once the move is complete? DNS or otherwise?
You can't rename an Exchange server. You have to give it its proper name before installing Exchange.
