I ran filemon on the server
using IE, browsed to the /oab URL
even tried running test-outlookwebservices
no smoking gun showed up in filemon.

Download filemon.exe and run it on the server.
Make the request to the OAB virtual directory with the IE... if you get an 401, probably filemon.exe will show you in wich file you are getting Access Denied.

reinstall IIS, reinstall CAS role, configure certs...etc.

I see the 401 Unauthorized error which leads me to beleive this could be a permissions issue. Have any changes been made to IIS?

Outlook 2007 clients cannot download the OAB ( as what's supposed defined in the autodiscovery.xml file) this is determined by the output of the 'test email autoconfiguration' feature in Outlook 2007.
Is there a way to get this back?
