We have Exchange 2003/SP2. If a user's mailbox size is say 300MB
and this person deletes 200MB worth of emails, will the priv1.edb
file actually shows a decrease of 200MB. If I go to the folder that
contains this priv1.edb file, will I actually see this decrease in
real time?

No, but you'll see more free space when you check your event IDs 1221
(online defrag/maintenance, after it runs).
You would have to do an offline defrag using eseutil in order to
shrink the actual database file size, but there is generally no need
to do this (it is not meant to be run as part of standard
maintenance). Exchange will happily reuse the whitespace.
