You don't use Exchange to encrypt, you use an email client to encrypt.
The individuals have their own x.509v3 certificates they use to
digitally sign, or encrypt, each message.
To encrypt a message the sender must have access to the public key of
the recipient(s). The recipients mus, in turn, have access to the
public key of the sender.
The 3rd-party, in this case, is the Certificate Authority. You can run
your own CA, but the cost to obtain a certificate that allows you to
creae your own trusted certificates is usually pretty expensive.
