So, from outside your network, can you telnet on port 25 to your no-ip
account/host and get your Exchange banner?
When you ping your no-ip account/host does it return the correct public IP?
Did you add myrealdomain.com to your recipient policy in Exchange, and are
all your mailboxes correctly getting stamped with the right email addresses
on the domain?

Note that since you're using a dynamic public IP, you're likely to have
difficulty delivering outbound mail to many servers outside your
organization - I suggest you forward your outbound Internet mail to your
ISP's SMTP server as a smarthost, in an SMTP connector.

The ping reply is accurate and I can nslookup on it also and get the
correct response. Is there an online telnet tool that will allow me
to test it? I have tried it before, but want to make sure that it is
available from others.

I have added the realdomain.com to the Recipient policy.

I have no problem at all sending e-mail externally..
